당신은 초음파 언어를 구사할 수 있는가?
수년 전 초음파 유도 통증 치료 시술에 관한 기사가 통증 치료 저술에 등장하기 시작했을 때 처음 든 생각이 “기사 거리가 이
것 밖에 없을까?”였다는 사실을 인정해야만 했다. 당초 이러한 생각을 하게 된 것은 내가 가장 잘 알고 있는 분야인 표식
(landmark) 기반, 형광 투시, CT유도 시술에 대한 개인적 애착이 크게 작용했기 때문일 것이다. 그러나 정말 솔직하게 얘기한다
면 통증 치료 시술 유도를 위한 초음파 활용에 대한 나의 초기 거부감과 회의는 초음파 영상을 제대로 읽을 수 없었기 때문인
것 같다. 초기 초음파 영상 중 다수는 최적의 품질과는 거리가 멀어 나 역시 그 영상을 판독할 수 있는 능력이 없었고, 다른 통
증 치료 전문의와 마찬가지로 “초음파 언어”를 아직 배우지 못했다는 단순한 사실 때문인 것이다. 촬영 영상들이 “의미하는”
바를 이해하지 못했던 것이다.
Philip Peng, Samer Narouze, Andrew Grey를 비롯하여 초기에 초음파 기술로 전향한 많은 전문가들의 노력 덕분에 나는 초음파라
는 새로운 언어의 기본 어휘부터 배우기 시작했다. 시작은 아주 어려웠고 초음파 영상이 “의미하는”것의 상당 부분을 놓치기
도 했다. 그러나 시간이 지남에 따라 어휘에서 한 발 나아가 대화의 상당 부분을 이해하는 수준에 이르렀고, 그 결과 초음파 분
야가 표식 기반, 형광 투시 분야만큼이나 아주 풍부하고 많은 함축된 의미를 담고 있는, 다르면서도 아주 흥미로운 분야임을 알
게 되었다.
나는 여전히 초음파 언어를 계속 배우고 있는 단계지만 본 교재를 통해 통증 치료 의학 분야에 새로 합류한 이 흥미로운 영역에
관한 소견을 제시하고, 초음파 언어가 왜 배울만한 가치가 있는 분야인지에 대해 설명하고자 한다.
1.초음파를 활용하면 진단의 정확성을 높일 수 있다. 초음파는 임상의에게 통증 부분의 실시간 스냅샷을 제공할 뿐만 아니라 모
든 주요 병력 및 물리적 검사를 통해 파악한 결과를 실제적으로 활용할 수 있는 진단 정보를 제공해 준다는 점에서 아주 독특하
다. 신경 포착(nerve entrapments), 체액 집적(fluid collections), 건염(tendinopathy), 감염(infection), 이물질(foreign
bodies), 관절염(arthritis) 등을 모두 초음파를 통해 쉽게 관측할 수 있다. 초음파는 또한 임상의로 하여금 문제가 되는 부분
의 해부학적 구조의 동적 관측을 가능케 하는데, 예를 들어 이두근이 굽혀지거나 늘려질 때 이두근 힘줄 탈구(bicipital tendon
sublux), 이상근(piriformis muscle)에 의해 포착된 좌골 신경(sciatic nerve) 등의 관측을 가능케 한다. 이러한 실시간 동적 정
보의 제공을 통해 보다 정확한 임상적 상관 관계 및 진단이 가능하며, 궁극적으로는 더 나은 안전한 치료 방법의 도출이 가능해
진다.
2.초음파를 활용하면 더 나은 치료가 가능하다. 초음파 유도 통증 치료 시술에 관한 많은 찬사는 “주사 바늘을 보다 정확한 위
치에 삽입”이 가능토록 한다는 점에 집중되고 있다. 많은 통증 치료 시술에서 초음파 유도를 통해 더 정확한 위치에 주사기를
삽입할 수 있음은 전혀 의심의 여지가 없지만, 잘 알려지지 않은 진정한 도움은 바로 초음파 검사를 통해 취득한 정보가 임상의
가 주사를 삽입하지 말아야 할 통증 조건을 알려주는 경우이다. 상당한 급성 염증과 힘줄 파열(tendon tear) 상태인 심각한 건
염 발생의 경우 주사기를 약하게 삽입해도 힘줄이 파열될 수 있는 위험이 있는 경우가 바로 그 예이다.
3.방사선을 사용 할 필요가 없다. 많은 통증 치료 전문가는 일상적인 형광 투시 사용이 미치는 상당 위험에 익숙하다. 납 앞치
마, 납 안경 등을 착용해야 하는 불편함이 없는 것은 말할 것도 없고, 건강상 위험을 회피할 수 있다는 것은 아주 좋은 장점이
다.
4.초음파는 교육에 활용하기 좋다. 진료실, 병상, 수술실에서 초음파 기기를 손쉽게 활용하게 되면 의료진, 레지던트, 의대생에
게 많은 시술 과정을 교육할 수 있다. 성상 신경절 블록 시행 시 회전근 개(rotator cuff) 기능 해부학의 동적 영상이나 샤쎄냑
결절(Chassaignac tubercle)과 경동맥(carotid artery)의 관계를 보여주는 것은 의대생들이 통증 치료 시술 시행 시 알아야 할
것을 학습하고 기억하는 데 도움을 준다. 심지어 경험 많은 통증 치료 시술 전문의에게 주사기가 얼마나 쉽게 잘못 삽입될 수 있
는지 상기시켜 줄 수도 있다.
5.초음파 장비는 계속 진화한다. 본 교재의 디지털 영상과 초기 통증 치료 저술의 초음파 영상만 비교해 보아도 이를 쉽게 이해
할 수 있을 것이다. 신세대 초음파 기기는 무한정 개선된 품질의 영상을 생산해 낼 뿐만 아니라, 보다 가볍고, 신뢰할 수 있는
기기로 발전하고 있으며 저렴한 것들도 가격 역시 저렴해지고 있다.
이 저서를 통해 환자의 고통 경감이 본질적 존재 목적인 초음파 언어에 대해 배우고 이해할 수 있기를 희망한다. 마지막으로 한
가지 질문을 하겠다. 당신은 초음파 언어를 구사할 수 있는가?

어 영광으로 생각합니다. Waldman 선생님은 통증분야의 저술에 있어서 탁월한 저서들을 쓰신 분입니다. 초음파 영역에서도 역시
뛰어난 저서를 발간하여 원저 역시 best seller로서 많은 선생님들께 사랑을 받았습니다. Best seller로서 많은 사랑을 받은 책
을 번역하여 세상에 내어 놓자니, 마음에 부담감이 많이 듭니다. 비록 부족한 부분도 있겠지만, 너그러운 이해와 애정 어린 질책
으로 사랑해 주시길 간곡히 부탁 드립니다.
초음파는 통증치료에서 이제 선택이 아닌 필수 장비가 되어가고 있습니다. 통증분야에서의 초음파의 역할은 사용의 편리성과 안
전성에 있다고 할 수 있습니다. 근골격계 분야의 진단 및 초음파 유도하 치료까지, 현재 초음파 장비의 영역은 점점 넓어지고 있
습니다. 본 역서가 통증치료에 전념하시는 여러 임상 의사 선생님들께 미력하나마 작은 도움이 되길 희망합니다.
